Almost all UnitedHealthcare® Medicare Advantage members have the UnitedHealthcare UCard™.* Member ID cards will state either HMO or PPO after the plan name but before “with dental”.

Your participation in the UnitedHealthcare Dental National Medicare Advantage Network allows you to see members in both HMO and PPO plan types. You will get paid for your services in HMO and PPO plan types in the same way:

*Peoples Health Plans in Louisiana use a different member ID card. To verify eligibility, look for “with UnitedHealthcare Dental” after the plan name and call the number on the card with any questions.

**You will be paid on different fee schedules if you participate in the UnitedHealthcare Dental Dual Complete Network for Arizona, New York or Pennsylvania.
