New bill of sale document

The new bill of sale document will be generated and completed in our provider portal. This will replace our current delivery receipt and must be completed on the day of the fitting.

Steps to access the bill of sale document

• Log in to the UnitedHealthcare Hearing Provider Portal
• Click on 8. Bill of Sale tab, fill in required fields and save
• Generate and sign bill of sale
  • Patients without email: You’ll need to print and provide them with a completed copy
  • Patients with email: They’ll automatically receive a copy via secure email
