As a reminder, UnitedHealthcare Community Plan of New Jersey covers Avastin (J9035, Injection, bevacizumab, 10 mg), for retinal conditions as described below:

• J9035, Injection, bevacizumab, 10 mg, when billed in a non-facility setting and billed with CPT code 67028 (Intravitreal injection of a pharmacologic agent (separate procedure) by an ophthalmologist or ophthalmology subspecialist
• C9257, Injection, bevacizumab, 0.25 mg, when billed in a facility setting
