New Jersey Medicaid: Verify practice technology use

Per New Jersey state regulations, we’re required to report quarterly how providers use technology.

Please help us learn more about your practice and how you use technology by completing the form emailed to you from our Provider Demographics team.

How to submit information

When you receive an email from our Provider Demographics team, from either provider_demographics@uhc.com or adobesign@adobesign.com, please complete the form and hit “Click to Sign” to submit.

The form includes questions regarding your practice’s experience or participation with:

• Certified Electronic Health Technology (CEHRT)
• Health Information Exchange networks
• CMS Promoting Interoperability Program (formerly Medicaid or Medicare EHR Incentive Program)
• Diabetes prevention programs
• NJ Vaccines for Children program
