Refer at-risk patients to nutritional program

Pennsylvania Medicaid members with acute illnesses can receive free, delivered meals on a temporary basis

UnitedHealthcare Community Plan of Pennsylvania network health care professionals can refer members who are in acute nutritional risk due to a serious illness to receive free, delivered meals through the MANNA food program. 

Members who have been diagnosed with the following acute illnesses may be eligible for the program: 

  • Diabetes
  • Chronic heart disease
  • End-stage renal disease
  • HIV/AIDS

To qualify, members must have visited the emergency room 1 to 2 times in the current year, and/or have been admitted to the hospital at least once during the year for their diagnosis. Members approved for the program will receive 3 meals a day, 7 days a week for the time allowed. The duration of the program is based on the individual’s diagnosis. In addition, a licensed dietitian will provide 2 counseling sessions by phone. 

How to refer members

If you have a patient who may be eligible for this service, complete the referral form for review by the MANNA Nutrition and Client Services team. Review the flyer for additional information.
