Earn incentives as a Patient Centered Medical Home

UnitedHealthcare Community Plan of Michigan’s Primary Care Provider (PCP) Pay-for-Performance Compensation program includes incentives for Patient Centered Medical Home (PCMH) certified practices.

If your practice is PCMH certified, it gives you the opportunity to potentially earn a higher monthly care management fee.

UnitedHealthcare Community Plan accepts the following PCMH designations as part of this program:

  • National Committee for Quality Assurance (NCQA®)
  • Utilization Review Accreditation Commission (URAC®)
  • Accreditation Association for Ambulatory Health Care (AAAHC) Medical Home
  • The Joint Commission® Primary Medical Home
  • Commission on Accreditation of Rehabilitation Facilities – Health Home (CARF)
  • Blue Cross Blue Shield of Michigan Primary Group Incentive Program (PGIP)
  • Other Michigan Department of Health and Human Services (MDDHHS) approved certifications

How to earn

To receive PCMH practice payments, practices must fax a copy of the active, not-expired, PCMH certification from the accrediting entity to UnitedHealthcare Community Plan of Michigan at 844-304-2840 by Nov. 11, 2024. 

Failure to fax this information will result in losing your current, if any, care management fees for your practice.

UnitedHealthcare Community Plan of Michigan will also be reminding practices to send in a copy of the PCMH certification by mailing to each practice.

Designation status for PCMH practice payments updates occur in January and July each year. Payments exclude Children’s Special Health Care Services (CSHCS) and Medicaid Secondary members.
