New UnitedHealthcare mailing address for contract notices and network information

Starting Jan. 11, 2024, you’ll mail all written notices required by your Participation Agreement to a new UnitedHealthcare mailing address. However, all informal contract inquiries can be answered using our UnitedHealthcare Provider Portal chat feature, or by mailing requests to a different address.

Where to send information

When sending contractually required written notices by certified or overnight mail, please send information to the following address:

Please do not use this address for claims remittance. You’ll continue to use the address on the back of the member’s ID card for that.
