New Jersey: Complete survey to avoid credentialing denials

Effective Nov. 1, 2024, as stated in section 4.8.5.C of the UnitedHealthcare Community Plan of New Jersey contract, you must send us a completed Special Needs Survey form with your initial credentialing application.

Who must submit this information

All care providers submitting initial credentialing applications must complete and submit the survey form. If you do not submit a completed form with your initial credentialing application, it will be denied until we receive the completed survey form.
