New Jersey: Complete survey to avoid credentialing denials

Effective Nov. 1, 2024, as stated in section 4.8.5.C of the UnitedHealthcare Community Plan of New Jersey contract, you must send us a completed Special Needs Survey formopen_in_new with your initial credentialing application.

Who must submit this information

All care providers submitting initial credentialing applications must complete and submit the survey form. If you do not submit a completed form with your initial credentialing application, it will be denied until we receive the completed survey form.
