Surest plans: Change to oncology prior authorization process

Effective July 1, 2024, for Surest™ plans and Surest Flex plans, we’ll require that you submit prior authorizations for outpatient chemotherapy and radiation therapies using the Optum® Cancer Guidance Program (CGP), similar to other UnitedHealthcare plans. This applies only to patients who have been diagnosed with cancer.

Your office may already be familiar with this process if you submit prior authorizations for these services for other UnitedHealthcare plans. CGP has a high provider Net Promoter Score due to its easy-to-use, intuitive platform and quick approval time.

If you need to modify an existing regimen

If you need to modify an existing regimen or treatment plan for which we already granted prior authorization approval before July 1, we’ll require you to resubmit your prior authorization request to us.

How to request prior authorization using CGP

First, sign in to the UnitedHealthcare Provider Portal to access the Prior Authorization and Notification Tool. Then, you’ll be directed to the Optum CGP tool.

  1. From any page on UHCprovider.com, select Sign In at the top-right corner.
  2. Enter your One Healthcare ID
    a) New users who don’t have a One Healthcare ID: Visit the Provider Portal registration page to get started
  3. From the left-hand tabs, select Prior Authorizations & Notifications. Then, click “Create a new request.”
  4. Select Oncology and enter the member’s state and plan details. You’ll be directed to the Optum website to complete your request.
