Kentucky providers: Information about Medicaid benefit updates

Changes affect Passport by Molina Healthcare and UnitedHealthcare Community Plan Medicaid members

June 2023

As of Jan. 1, 2023, benefits were updated for Passport by Molina Healthcare and UnitedHealthcare Community Plan Medicaid members in Kentucky, reflected in the tables below. Please note, we’ve added additional clarification around progressive lenses as they are covered by Passport by Molina Healthcare.

Passport by Molina Healthcare:

Benefit Updated benefit limitations/criteria
Eyeglasses
  • Adults 21 and older no longer have an allowance for eyewear
  • Adults 21 and older are now eligible for 1 pair of eyeglasses every calendar year when criteria is met
Progressive lenses;
  • 2 units of progressive lenses allowed per calendar year

UnitedHealthcare Community Plan:

Benefit Updated benefit limitations/criteria
Eyeglasses
  • Adults 21 and older are now eligible for 1 pair of eyeglasses every calendar year
Eyeglass replacements
  • Adults 21 and older are now eligible for 1 pair of replacement eyeglasses every calendar year, when criteria is met
Medically necessary contacts
  • Adults 21 and older are now eligible for medically necessary contact lenses in lieu of eyeglasses when criteria is met
