Effective Jan. 1, 2025, UnitedHealthcare Community Plan will no longer be a participating plan in our UnitedHealthcare Community Vision Network / March Vision Network. UnitedHealthcare Community Plan will not be a Minnesota Health Care Programs option for members in 2025 (Families and Children, Senior Health Options and Senior Care Plus, and Special Needs BasicCare). The health plan has notified all members impacted by this change.
Members were encouraged to select a new health plan by Dec. 6, 2024, for an effective date of Jan. 1, 2025. If they choose not to do so, a health plan will be selected for them by the Minnesota Department of Human Services (DHS) to ensure a smooth transition of care. If your patients have questions about selecting a new health plan, they can call the Minnesota Department of Human Services (DHS) Health Care Consumer Support by Dec. 6, 2024, at 651-431-3722 or 833- 970-0047.
Eligibility and benefits: Please continue to verify member eligibility on providers.eyesynergy.com when a member is seeking vision care.
Claims submission: Continue to submit claims for eligible members with dates of service up to and including Dec. 31, 2024. UnitedHealthcare will remain responsible for all vision services rendered to members through Dec. 31, 2024, at 11:59 p.m. Effective Jan. 1, 2025, the new health plan will be responsible for the members’ care, claims and authorizations. Please ensure all claims are submitted for payment within 180 days of the date of service.
Member questions: Members can call the health plan at 888-269-5410, TTY 711, 8 a.m.-6. p.m. CT, Monday–Friday. Member services staff will continue to be available through June 2025.
Continuity of Care: Please call the member’s new MCO beginning Jan. 1, 2025, to understand their continuity of care procedures. UnitedHealthcare coverage for all members ends Dec. 31, 2024.
Disputes and appeals: Our provider dispute and appeal process and member appeal and grievance process will remain the same.

It contains information about the process for submitting appeals and other requests on behalf of members.
