New Empire Plan mailing address for contractual notices
Starting on Sept. 1, 2023, there’s a new mailing address for written notices required or permitted by The Empire Plan participating provider agreement.
When sending contractual written notices by mail, health care professionals directly contracted with The Empire Plan need to use the following address:
