Patient Care Opportunity Report (PCOR) is a monthly report you can use to identify your patients’ adherence to preventive screenings, treatments and medications.
In the coming months, you will access your PCOR via Practice Assist using your One Healthcare ID. If you do not have a One Healthcare ID, you can create one on the UnitedHealthcare Provider Portal or by calling 855-819-5909.
This transition will continue to simplify the user experience by moving the PCOR reports into Practice Assist, a tool you can use daily to manage your patient populations. Additional communications will be forthcoming with the specific timing of this transition.
To request access to Practice Assist, submit an email request to the Practice Support team at rqns_prospective_support@optum.com. Please include your first and last name, Tax ID Number (TIN), provider group name and health plan. Please allow 7 business days for us to complete your access. We'll send you a welcome email once we've completed the process.
