Influencing the best outcomes for diabetic patients

This is a comprehensive provider and member engagement program to influence the best outcomes for diabetic patients. You can become certified in our Clinical Care and Coordination Program, which includes a directory designation with a badge displayed by your name.1

The program also includes exam reminders for patients and ongoing education opportunities. You’ll be eligible for certification once you meet our established clinical and quality program criteria. 

Program certification criteria

  • Must be an active provider with UnitedHealthcare Community Vision Network / March Vision Network for exams and materials
  • Complete the required training courses
  • Perform dilation/retinal imaging on diabetic patients during comprehensive exam
  • Send notifications with exam outcomes for every diabetic member to their primary care physician 
  • Submit CPT®II codes on vision claims to UnitedHealthcare Community Vision Network/ March Vision Network
