Louisiana Peoples Health Special Needs Plans
Peoples Health Dual Special Needs Plans (D-SNP) are HMO-POS plans that offer benefits for people with both Medicare and Medicaid. These plans provide benefits beyond Original Medicare, such as transportation to medical appointments and routine vision exams, and none require referrals. Members must have Medicaid to enroll, although the level of Medicaid required varies by plan.
- Peoples Health Secure Health (HMO-POS D-SNP): Must have Medicaid
- Peoples Health Secure Complete (HMO-POS D-SNP): Must have cost-sharing assistance under Medicaid
- Peoples Health Dual Complete LA-S5 (HMO-POS D-SNP): Must have full Medicaid
Peoples Health Chronic Condition Special Needs Plans (C-SNP) are HMO-POS plans that provide specialized benefits and services for Medicare beneficiaries with chronic conditions such as diabetes, heart failure and/or cardiovascular disorders. These plans do not require referrals for specialty care.
- Peoples Health Complete Care LA-5 (HMO-POS C-SNP)
- Peoples Health Complete Care LA-6 (HMO-POS C-SNP)
- Peoples Health Complete Care LA-7 (HMO-POS C-SNP)
Health Maintenance Organization (HMO) plans have a defined network of contracted local physicians and hospitals to provide member care. Generally, members must use these care providers to receive benefits for covered services, except in emergencies.
Point of Service (HMO-POS) plans include all the features of HMO plans plus the ability to go outside the contracted network for certain health care services - typically at a higher cost. Peoples Health POS plans only cover out-of-network dental care.
