Apply to be part of our Preferred Lab Network

Consider applying to be part of the UnitedHealthcare Preferred Lab Network, which would be effective on July 1, 2025. The Preferred Lab Network includes currently contracted, freestanding laboratories that have met high standards for access, cost, data quality and service. 

Benefits

If your application is approved, you’ll receive the following benefits:

• We’ll highlight your lab in our directories to make it easy for members to access care at your lab. This also helps health care professionals to send referrals to your facility. 
• We’ll provide you with approved language to be able to promote this designation in your marketing materials

To apply, request an application by emailing us at preferred_lab@uhc.com by Jan. 15, 2025, with Preferred Lab Network in the subject line.

In the body of the email, please provide:

• Contact name, position and email address
• Complete mailing address
• Organization name
• Organization TIN
• National Provider Identifier

We’ll email you the link to the application, which you’ll need to complete by Jan. 31, 2025.

If your lab meets the criteria, we’ll let you know by May 30, 2025. We’ll also send you an Amendment to your Participation Agreement to include the terms associated with your Preferred Lab Network status.
