Arizona: Make demographic updates quickly with online tools

UnitedHealthcare Community Plan of Arizona would like to remind you of the following tools available for submitting your demographic updates. Please note, failure to submit demographic changes may result in inaccuracies in our directory, a negative patient experience and delays in claims payment. 

• Council for Affordable Quality Healthcare (CAQH) Provider Data Portal: Access directory services and update profiles in real time. The data portal is designed to serve multiple payers, meaning any updates or attestations you make will be accessible to participating payers you’ve authorized.
• My Practice Profile: The My Practice Profile tool on the UnitedHealthcare Provider Portal lets you view, update and attest to the care provider demographic information UnitedHealthcare members see for your organization. Benefits and features include but are not limited to:  
  • Provider accepting patients/panel status
  • Add and remove physicians from your TIN
  • Office address(es) and hours of operation
  • Phone, fax, email address and website
