UnitedHealthcare national Gold Card code update

The UnitedHealthcare national Gold Card program rewards contracted provider groups who consistently adhere to evidence-based care guidelines. The Gold Card program applies to all UnitedHealthcare commercial, Individual Exchange*, UnitedHealthcare® Medicare Advantage and UnitedHealthcare Community Plans.

The national Gold Card code list has been updated effective March 17, 2025. During our program performance review, we identified codes not utilized by our contracted provider groups and have made the necessary updates to remove the codes from the code list. The updated Gold Card code list is posted along with all other resources on our national Gold Card program information page.
