Virginia Medicaid: Complete an OBRAF to earn a $100 incentive

Earn incentive dollars for your practice by completing a Virginia Obstetrical Risk Assessment Form (OBRAF) for your pregnant patients who are UnitedHealthcare Community Plan of Virginia members. For each form you complete and submit within 10 days of a member’s first prenatal appointment, you’ll earn $100.

Submitting an OBRAF

There are 2 ways to complete and submit the OBRAF:

• Complete the OBRAF online by signing in to the UnitedHealthcare Provider Portal
      1. Go to UHCprovider.com and click Sign In at the top-right corner
      2. Enter your One Healthcare ID
      3. Select Clinical and Pharmacy and then Care Conductor and Notification of Pregnancy
• Download and complete the OBRAF, then fax it to 877-353-6913

One OBRAF incentive is allowed per patient. We’ll pay incentive dollars quarterly. You do not need to submit a claim to receive your bonus.

Comprehensive care and support for members

Our case managers review each form received and will engage with the member to provide education, resources and support. All pregnant members will be enrolled in our Healthy First Steps™ maternal care program. Healthy First Steps provides members with education, tools and touchpoints for healthy behaviors. The program supports your plan of care while helping to meet HEDIS® clinical standards. We’ll also assign members identified as high-risk a dedicated nurse coach to support them during their prenatal and post-partum care.
