Clinical Care and Coordination Program
Our Clinical Care and Coordination Program is a comprehensive provider and member engagement program designed to influence the best outcomes for members with diabetes through on-going provider education opportunities, billing guidelines and member outreach.

Eligibility
You are eligible for certification once you meet the following clinical and quality program criteria.
Tier 1
- Must be an active provider with UnitedHealthcare Community Vision Network I March Vision Network for exams and materials
- Complete the required training courses:
- Perform dilation/retinal imaging on diabetic patients during comprehensive exam
- Send notifications with exam outcomes for every diabetic member to their PCP
- Submit CPTII codes on vision claims to UnitedHealthcare Community Vision Network / March Vision Network
Tier 2
This certification level will be introduced soon.
- Complete and maintain Tier 1 criteria
- File 10 or more vision claims for diabetic members in a 3-month period
- Correctly submit CPTII coding on 75-95% of claims with members identified with diabetes
Tier 3
This certification level will be introduced soon.
- Complete and maintain Tier 1 criteria
- File 10 or more vision claims for diabetic members in a 3-month period
- Correctly submit CPTII coding on 96%+ claims with members identified with diabetes
Recognition
This badge will appear by your name in our provider directory to indicate you are certified in our Clinical Care and Coordination Program.
